# Chesapeake Connections

Supportive mental health services for people with mental illness and developmental disabilities.

Individuals who have mental illness and developmental disabilities find comprehensive, compassionate care at Chesapeake Connections. 

Our integrated team provides year-round care, including counseling services and medication therapy. We manage your or your loved one’s physical needs, address housing and employment issues, and provide other services.

## What Is Chesapeake Connections?

Chesapeake Connections delivers a wide range of treatment options and support services for people who experience severe and persistent mental illness (SPMI) and developmental disabilities, including:

- Schizophrenia
- Bipolar disorder
- Major depression that significantly interferes with thoughts, mood, and actions 

Our complete care includes counseling services and [Medication Management](https://johngeorgepsychiatrichospital.org/encyclopedia/medication-management/). We provide individualized care, meeting each person’s needs and helping them maintain success in their community. 

We offer:

- Mental health counseling
- Medication monitoring
- Access to physical health care 
- Support in finding employment and housing

## Who We Serve at Chesapeake Connections

Through the right combination of mental health services and support programs, we help people achieve success in their community. 

We work with people who have one or more serious and persistent mental illnesses *and* have received previous psychiatric treatments:

- Admitted to a state psychiatric hospital for six or more consecutive months
- Admitted to an inpatient psychiatric unit at least four times in the past two years
- Visited an emergency department for psychiatric reasons at least seven times in the past two years
